Perl file download zero bytes

Both servers have 4gb memory and have been losing total cache buffers since sp3. The swap files show 0 bytes leaving me to believe the virtual memory is failing to optimize by swapping to disk. Space is not a concern with at least 2gb free on every volume. Any help would be appreciated. Today one of our users complained that they had saved a file but could not open it. After looking into the problem we found that the file name was saved in their home directory but it had a 0 bytes files size.

They apparently were working in MS Word and saved the file directly to their home space. Having done a search on our backups system it appears that this is not a single occurence.
